James Bond est en petite forme et les derniers tests qu'il a passés ne lui sont guère favorables. En conséquence, le chef des services secrets britanniques, «M», dont il a perdu la confiance, l'envoie se refaire une santé à la clinique Shrublands, près de Londres. C'est là qu'il surprend, une nuit, un affrontement entre le capitaine américain Jack Petachi, qui vient d'être opéré de l'oeil droit, et sa jeune et belle infirmière, Fatima Blush. Mais cette dernière, qui est en fait un agent de la redoutable organisation criminelle du Spectre, finit par identifier 007. Impliquée dans une opération d'envergure visant à s'emparer de deux ogives nucléaires américaines, elle n'a pas l'intention de laisser celui-ci perturber ses plans…
